How it Works
Hope and Life provides a combination of Level 1 and Level 2 Support Coordination.
Our Support Coordinators work with participants and their loved ones to navigate the NDIS by providing the necessary information to be able to make informed decisions around their supports. We inspire confidence by recognising the unique needs and aspirations of each individual, assisting with the processes around creating a personalised plan that enables them to meet their goals. With the purpose of fostering independence, we collaborate with participants to cultivate their capacity and resilience so that they can be empowered to direct their own lives.
